先週に引き続き23例報告．1回目のアストラゼネカワクチン接種後6-24日後の血栓症・血小板減少．
22例でHIT抗体陽性．ヘパリン使用はなし．
フローチャートの案も書いてある．非ヘパリン系抗凝固とIVIGもいいんじゃないかとか．
あとHIT抗体も検査法によって偽陰性が出たりとか書いてあったがちょっと自分の知識で解読不可能．

あんまりせん妄・昏睡の部分メインで両者を使い分けないかな．そんなにICU例診てないからしっかりしたこと言えないけど．

リンパ節腫脹+不明熱ってなかなかそそられるパターンだけど，意外と鑑別が多いし詰め切れない．
MGHでは既に完璧な問診と初期対応が終わっているが，自力でも上がった鑑別から病歴を引き出して詰められるか．

判断根拠が乏しい，との記載ではありますが，とりあえず重要なのは「無症状ビタミンD欠乏患者にそれのみを理由にビタミンD治療を行っても，死亡率や骨折リスク，2型糖尿病発症に対するメリットはないという十分なエビデンスがある」という点かと．
あんまり骨折もないのにスクリーニングしようと思ったこともないけど．

84％のリスク減，再感染まで大体7カ月程度は保護的な効果がありそうとのこと．
逆に言うと7カ月で再感染一定数出ると．．ワクチンも繰り返しいる感じなのかなあ．
